To get a paperless W-2, first update your
contact preferences and activate your e-
mail address, if needed. Go to MyPay.
Select your initials on the top right-
hand corner of the window and
select Settings from the dropdown
menu. Navigate to the Go Paperless tile.
Select Go Paperless. Turn on your email
notifications and consent to receive
paperless statements. You will receive

401(k)
401(k) Account Balance: You can elect to receive
a distribution of your vested 401(k) account
balance through Fidelity

EAP and Nurse Advice Line Continuation
Coverage
As an active employee of Amazon, you received,
at no cost to you, employee assistance program
(or “EAP”) and nurse advice line coverage under
the Amazon.com Services, Inc. Health &
Welfare Plan (“Plan”). Upon termination of
employment, this EAP and nurse advice line
coverage will continue to be provided to you at
no cost for 90 days (“Continuation Coverage”).
Continuation Coverage will begin on the day
following your last day at Amazon and continue
until the last day of the 90th day following
your last day at Amazon.
Federal law requires that most group health
plans (including the EAP and nurse advice line
coverage offered under the Plan) give
employees the opportunity to continue their
group health plan coverage if there is a
“qualifying event” that would result in a loss of
coverage. Here, your termination of
employment is not an event that causes you to
lose Plan coverage. The Continuation Coverage
automatically provided to you meets
requirements under federal law and is provided
in lieu of coverage that would otherwise be
required. You will have no additional rights to
access EAP and nurse advice line coverage after
the end of the Continuation Coverage period.
