As an Admissions Coordinator (AC), I provide customer service to potential patients and their families, professional referral sources and Seasons staff to effectively and efficiently coordinate referrals and respond to inquiries around hospice services. I am a member of the admissions team which builds and maintains provider relationship. I obtain critical referral information to ensure timely response and data entry in the EMR system. With the goal of scheduling a sales visit and supporting their Families/caregivers. Bringing together the highest level of engagement between team members, patients, and their families. Giving them more attention, hospice services, helping navigate the emotions arising during this stage of life and discussing a fully funded hospice benefits through Medicare, most commercial insurance, Medicaid, and unfunded charity programs. I collect data from the field and input all the updated information that would help process the referral along. I make sure the information is updated regularly with accurate data. This is called “Intake activities”. To substantiate hospice admissions I receive the referrals, medical records and Insurance info by electronic form, paper/scanned HCHB, order print, file in chart and process them in the EMR platform system. Work with documents and patients that are VA PATIENTS, DNR, Proxy, Healthcare surrogate, Power of Attorney and Guardianship involvements.
