This trial position was an attempt to see if a PA could decrease the work load of three very busy cardiologists. Originally, my goals were to initiate Heart Failure Program clinic and start up Chest Pain Observation program. However, it became apparent that this would not be possible, due to the daily work load. I performed all the hospital treadmill stress, lexiscan stress and nuclear cardiolyte tests for Saint Joseph’s Hospital when any of my 3 physicians were on call (220+in 4-6weeks), in addition I did cardiology consults both in ER and on in-patients. I was working 10hr days, sometimes with 10 day stretches, but there was no way I could significantly unburden 3 different cardiologists with 3 separate practices. Therefore, the position was terminated, along with my plans for a CHF clinic & CP observation unit.

-Clinical supervisor of the nurse device clinic—managing any new arrhythmia discovered, trouble shooting device problem, reprogramming of defibrillators and pacemaker or approving changes to be done at f/u. Pre and post op device implant management, identify electrocardioversion pts, EP Consult evaluations, General Cardiology and Internal Medicine pt care. Director, Cardiology Out-Patient Service Administrative Responsibilities- Designed and implemented a multidisciplinary clinic to evaluate, educate, consent and schedule patients referred for out-patient invasive cardiology procedures. These procedures included cardiac catherization, ptca, electrophysiologic evaluation, pacemaker insertion, and electrocardioversion.

-Developed, facilitated and staffed a clinic for the cardiology section to care for their primary care patients who need immediate but not emergency care for a wide variety of medical problems.

-Established, and was the sole provider for a private –pay Cardiology Tricare

-Developed an evaluation assessment method for stress test technicians to pre-screen for risk factors in all patients referred for testing.

Clinical Responsibilities:

Primary Care Clinic: Maintained a panel of primary care patients, who are seen in a weekly clinic. Pacemaker Clinic: Interrogation, reprogramming and problem management of pacemaker patients.

Post Procedure Care: provide clinical evaluation, patient education and counseling before/after cardiac out-patient procedures..

Exercise stress testing: supervision of technicians and assessment of pt before, during and after procedure. Interpretation of test and implementation of appropriate treatment or referral for testing

In-Patient Cardiology Consult Service- participate as team member in conjunction with Cardiology staff attending and two upper level internal medicine residents. Responsibilities include both administrative coordination of testing, resident supervision as well as patient evaluations for presentation and discussion during formal bedside rounds.
