Home Health Physical Therapist

●Provide physician ordered physical therapy services that include assessment, planning, developing, and implementing individualized care plans to the diverse socioeconomic and cultural group of patients that live within the Ashtabula county.

●Common diagnoses include atrial fibrillation, congestive heart failure, COPD, joint arthroplasty, fractures, vertigo and etc.

● Evaluate home environments and assess the need for durable medical equipment to enhance patient safety and independence within the home. Educate patients and family on appropriate use of durable medical equipment to reduce risks of injury.

●Well versed in OASIS assessment tool for home health care. Up to date on all necessary training courses for OASIS scoring and interpretation of scoring. Can manage/supervise a caseload of 30 patients and ensure that documentation is timely, accurate and complete.

●Consistently communicate with physicians and interdisciplinary team regarding patient response to therapy, renewing orders, verbal orders and discharge summaries. Delegate tasks and manage care plans in association with physical therapist assistants.

●Actively participate in monthly interdisciplinary staff meetings and agency committees that promote team development and patient centered care. Committees include patient education, infection control, acute care documentation and therapy department meetings.

●Demonstrate high standards of patient Code of Ethics and HIPPA privacy policies and procedures.

LEADERSHIP

●Earned commendations from agency director as a “Distinguished Caregiver” and also earned the “Quality Star Award” for outstanding documentation and patient care in 2018.

●Created a therapy research committee to continuously update therapists on the most current evidenced based literature in home health. Also developed an incontinence management handout that is utilized by home health staff.

●Member of the Leadership LIVE program at Thomas Jefferson University: A series of workshops, special events and community service opportunities are offered to enhance leadership development for health care professionals

●Was the president of the Health Science Club at Lock Haven University for 2 years: With this student organization I accumulated over 400 hours of community service and was recognized as Lock Haven's Most Outstanding Student Leader on Campus at the annual leadership awards ceremony.
