Making sure applicants get the help they need by making sure i am educated on what is best to get the applicant the assistance and help for them. I worked with many different systems to submit applications to be reviewed for final review and helped get assistance processed. Also did frequently well checks on clients and their families. Due to hippa made sure all document and information was kept private, as i traveled weekly to check on my clients. I also was able to build a strong relationship with my clients so that they can build trust and honesty with me. I scheduled caregivers and direct service workers for shifts for clients in need of assistance, some clients were medicaid clients. I answered phone inbound called and made outbound calls as well, i would remove caregivers and/or direct service workers from scheduling. I would hire, interview, speaking through orientation, getting caregivers and direct service workers trained along with cnas. I also completed home visits. I used systems such as outlook, global, ringcentral, paylocity, edge, microsoft word, microsoft teams, axiscare, conducted background checks, drug test and first aid-cpr. I conducted follow ups on clients and caregivers/direct service workers. I also have experience in training on lifts for bed bound clients, filing/submitting paperwork and doing home visits/wellness checks. I received an offer promotion and after less than 2 months being a care manager i was able to become part of the integration team. My new title is integration associate. What i do is because i am a highly motivated and dynamic integration associate i was able to join the team. This role was responsible for traveling to our newly acquired offices and ensuring the smooth integration of their operations into our system and processes. The integration associate play a pivotal role in training the new care managers and caregivers, thereby ensuring a seamless transition. I was able to travel to different states such as tennessee, louisiana, mississippi, and arkansas. I help with icd 10 codes being entered. I received a company card as well.

I went to elderly apartment complex, and signed clients up for Long Term, CCW. If they didn't have Medicaid, I would assist clients with their Medicaid Applications. I also went to Nursing Homes. if clients wanted to leave out of the Nursing Homes and go home, I would apply for a transition Waiver for them to go into an apartment or their family homes. If a parent or parents had a disabled child and didn't have services, i would assist them with a Capital Area application so they could get their child on the Now Waiver.
