Duties and Responsibilities

●Primary responsibility of researching, investigating, and reporting cases of communicable disease in the statewide Covid-19 response

●Provide professional guidance in carrying out objectives of the mission

●Interviewed patients and contacts to establish their case history and obtain contact information by applying excellence in motivational interviewing, cultural competency, and case management software and mailed contacts their results notification

●Discuss symptoms onset, exposure dates, and referrals to health care services and resource coordination

●Record and establish the description and timeline of onset of symptoms and interviews case to determine all possibly exposed individuals

●Explain information and recommendations regarding isolation and quarantine parameters, services available to case, and monitor case for ongoing needs and disease clearance

●Help case develop an appropriate plan which may include referral to another agency or professional

●Maintain detailed records of cases and assemble into periodic reports via data entry and proficiency with computer software programs
