SUMMARY:
I have 16 years’ experience in Mental Health and 10 years in Case management. Recently retired from CW Bill Young Bay Pines Veterans Medical Center in Bay Pines, Florida. As a MHICM RN CM with a caseload of 15 patients via telephone and face to face visits 2-3 times a week. During Covid period of 3/20 – 2/22 most visits were telephonic, for everyone safety. Previous employed at North Florida Randall Malcolm Veterans Medical Center, in Gainesville, Florida under position of (PCMHI) Primary Care Mental Health Integration Care Manager which telehealth was provided on a caseload of 90 Patients followed 3-4 times a month. While employed at WJB Dorn Veterans Medical Center, Columbia SC, I collaborated on several committees including Healthcare Failure Mode and Effects Analysis (HFMEA) which was conducted at Dorn VAMC relating to "The Process for Patient Transfers" and I participated in two RCA's teams in 3 suicides in 2012, 2015, and 2017. Active on "Suicide Risk Reduction Committee" from 1/2012 until 9/2018. Certification in Suicide Prevention from the United States Active Army and National Guard training program "The Assist". A 2-day training class on working with Active Suicidal people. It trained how to identify, de-escalate, and transition them to safety. Licensed with the State of Florida Medication Administration for new and renewals certifications for local residential facilities operating in State of Florida for the past 14 years.

QUALIFICATIONS
Worked in the following Mental Health Areas: Mental Health inpatient care 5 years (SHANDS of Jacksonville). Veterans Medical Centers positions of 2 years Case Manager for Mental Health Outpatient Care, while assisting for 2 years of inpatient care when needed, and 4.5 years of severe mental health conditions in program Mental Health Intensive Case Management (MHICM). Worked directly with severe PTSD, major depression D/O, MST, Anxiety, Bipolar, Schizophrenia, Schizoaffective and Personality Disorders. 1 year as Primary Care Mental Health Integration (PCMHI) Care Manager then transferred to return to MHICM Case Manager for Bay Pines VA to initiate and expand MHICM program in surrounding areas including Bradenton, Palmetto and Sarasota counties for 2.5 years. 9 years at level one trauma hospital with 15 years in the psychiatric nursing area 24 years in the healthcare field including financial, insurance and personal care. MHICM RN CASE MANAGER – SARASOTA / BRADENTON AREA
Assisted with MHICM LCSW to initiate and expand MHICM program in southern area of St. Petersburg MHICM program. Job included the areas of Bradenton, Sarasota, North Port, Nokomis, and Venice CBOCs. This included interviewing Veterans, surrounding Hospitals, ALF's, and Pharmacy's that assisted these Veterans with the medication in the past. The program started on 1/2/2020 and has increased to approximately 12 Veterans actively in MHICM at this time. Assisted Veterans to develop individual care plans includes SMART GOALS, and encouragement, and reminders for appointments and medication compliance. Meetings schedule for Psychiatric monthly appointments. All Veterans have shown improvement in Mental Health conditions with increased compliance with self-care.

Worked directly with Primary Care Physicians in the service area of Jacksonville, FL and St. Mary's, GA. Maintain a panel of 90 Veterans that have been started on MH medications or increased meds or changed meds, patients transferred from Psychiatrist back to Primary Care Provider. Patients are monitored for 4-6 months while patients become stable of psychiatric medications. Worked directly with Patient Aligned Care Team (PCP, Psychologist, SW, and Clerical staff). Patients followed by telephone and Video Cams. Care plans includes SMART GOALS, and encouragement, and reminders for appointments and medication compliance.

MH O/P: Worked directly with a patient in an outpatient capacity. Worked directly with psychiatrist and his case load of 1400 pts. RN responsibilities included any urgent or psychiatrist visits including medications interactions, case manager notes, and assistance with Psychiatrist when needed. RN would call for reminders and change appt dates if needed. RN will see patient prior to psychiatrist appt. Completed treatment plans, transfer paperwork, work notes, social security and disability paperwork.
MHICM: Worked with a staff of 10 team members, including Psychiatrist, SW, 5 RN's, Psychosocial Rehabilitation and Recovery Center (PRRC), and Suicide Prevention Team (SPT). Caseload of 18 pts with weekly visits of up to 3 hours per person. Assisted in CARF certifications (Mental Health Certifications) for 3 consecutive certifications (2012, 2015, and 2018).
