OBJECTIVE

I am a recent graduate with a Bachelor’s of Science degree in substance abuse counseling and I

am seeking a position as a Chemical Dependency Counselor. My LCDCIII licensure is in the

pending status on the OCDPB website & I have held a CDCA since 2010.

EMPLOYMENT HISTORY

Volunteers of America / Veterans Domiciliary: Wade Park Cleveland Ohio July 2013-Present

Residential Monitor: Responsible for a professional level of support for the veterans & the

supervision of veterans to ensure a safe and secure environment. Document all

activities/incidents of the veterans into the computer database. Conduct health and safety rounds

hourly.

Internship, McIntyre Center: Parma Hts. Ohio November 2009 to May 2011

Intake Coordinator:

Intakes: Conduct the initial intake process

Develop individualized treatment plans and sign consent forms with client.

Perform Screenings: Evaluate signs and symptoms of alcohol and other drug use, dependency

and abuse.

Assessments: Identify and evaluate client’s strengths, weaknesses, problems and needs for the

development of a treatment plan.
