Assisting Hands Home Care provides trusted in-home care for seniors and individuals who need help with daily living. We are a local, family-owned agency based in Jacksonville, and our small office team works closely together to ensure our clients receive dependable, high-quality care.

About the Role

We are seeking a full-time Care Coordinator to take ownership of caregiver scheduling and ensure clients receive the support they need, when they need it. You’ll manage daily schedule operations, respond to call-outs, staff new cases, and resolve escalations during business hours and while on-call. You’ll also work closely with our after-hours team to ensure they have clear instructions and support. This is a hands-on, detail-oriented role ideal for someone who thrives under pressure, enjoys problem-solving, and values teamwork. You will report to the Operations Director and be supported by a collaborative office environment.

Key Responsibilities

Own the day-to-day caregiver schedule, including shift assignments, call-offs, and time-off requests

Fill open shifts, staff new and existing cases, and manage last-minute changes

Communicate professionally and compassionately with clients and caregivers

Monitor the caregiver roster and work with Recruiting to ensure proper staffing levels

Review and update client care needs and match accordingly with caregiver qualifications

Coordinate with the after-hours team by preparing daily handovers and weekend plans

Serve as the first escalation point for the offshore team when issues arise after-hours

Work closely with the Operations Director and other team members on cross-functional activities

On-Call Responsibility

Prepare the after-hours team to resolve as many issues as possible before escalation

Serve as the primary on-call contact to handle urgent issues after hours when escalation is needed

Qualifications

Minimum 2 years of experience managing schedules or staffing in a healthcare or home care setting

Proven ability to manage multiple priorities under pressure

Strong communication skills and a courteous, solutions-focused approach

Familiarity with home care scheduling systems (e.g., WellSky) is a plus

Willingness to take full ownership of the schedule and be accountable for outcomes
