Home Health & Hospice RN position

Initial assessment of client and family to determine home health needs

Regularly re-evaluates client nursing needs

Initiates plan of care and makes necessary revisions

Develop care plan and establish goals based on diagnosis

Administers medications and treatments as prescribed

Provides health care instructions to the client

Identifies discharge planning needs

Acts as Case Manager when assigned

Prepares clinical notes and updates the primary physician

Communicates with the physician regarding the client’s needs

Ensures arrangements for equipment and services are available

Instructs, supervises and evaluates home health aide care

Job Requirements

Home Health & Hospice experience required

Must have current Driver's License & Auto Insurance

Schedule Information

8a-5p shifts

On call rotation for evenings/weekends

Holiday coverage required

Additional Details

BLS certification required

Service radius of 60 miles around Dickinson

Candidates may not be full time, part time, or PRN with any facility associated with CommonSpirit within the past year

RTO needed upfront; additional RTO will not be approved
