We are seeking a dedicated and experienced RN Case Manager to join our dynamic team. In this role, you will be responsible for coordinating patient care, managing complex cases, and ensuring that patients receive the highest level of care. Key Responsibilities:

Conduct comprehensive assessments of patientsâ physical, emotional, and social needs.

Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and healthcare teams.

Coordinate and manage care transitions, including hospital discharge planning and referrals to other services.

Monitor patient progress and outcomes, adjusting care plans as necessary.

Advocate for patients and their families, providing education and support throughout the care process.

Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure cohesive patient care.

Maintain accurate and up-to-date documentation of patient interactions, care plans, and outcomes.

Stay current with healthcare regulations, policies, and best practices to ensure compliance and optimal care delivery.

Manage a caseload of patients, prioritizing tasks and managing time effectively to meet deadlines and ensure quality care.

Work Schedule: Monday- Friday 8:00am - 4:00pm

Qualifications:

Current RN license in DC

Bachelorâ s Degree in Nursing preferred

Minimum of 2 years of nursing experience, with at least 1 year in case management or a related field

Strong clinical assessment and critical thinking skills

Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals

Proficiency in using electronic health records software

Ability to handle multiple tasks, prioritize responsibilities, and manage time effectively

Knowledge of healthcare regulations, policies, and standards

Flexibility to work occasional overtime or adjust hours as needed

Must Have The Following Update To Date Records:

CPR Card.

Hep B or Declination form

Flu shot or Declination form ( flu shot must have been administered within the last 12 months)

MMR

PPD or Chest Xray ( completed within the last 12 months ) ( if chest Xray within the last 5 years

T-dap (received within the last 10 years)

Varicella

Covid Vaccination (if johnson johnson both shots, if phizer both shots plus booster shot)
