Primary Care Provider / Family Nurse Practitioner (FNP)
Looking for an FNP to work as a Primary Care Provider in a clinic setting.
Pay: 73-74.00
Schedule:
Clinic hours, minimum 30/hrs per week. Requires working every other weekend and some major holidays
10-hour shifts M-F, 8 hours on the Weekends
Primary Care Family Nurse Practitioner (FNP) Requirements:
Completion of a masterâ s degree level Family Nurse Practitioner program with current National Board Certification
State of Employment license to practice in the Advanced Practice Nurse
A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner/Physician Assistant in either a family medicine or primary care practice preferred.
Primary Care Family Nurse Practitioner (FNP) Job Duties:
Uses knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional to provide care to patients
Performs history and physical assessments of patients that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical symptoms.
Provides patient counseling; inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication management
Counsels and educates patients and families on health maintenance and respond to patient care inquiries
Evaluates and treats patients' medical needs with safe and high-quality treatment
Documents all patient care within an electronic health record in accordance with current policies and procedures
Works as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patients
Provides patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease
Refers to appropriate specialists and patient care components as appropriate
Collaborates with clinical care team to ensure the delivery of high quality, patient centric, longitudinal care
Resolves conflict using appropriate communication and management techniques
Cultivates and maintains positive relationships with clinical care team members
