NOW HIRING: Community Health Action Team (CHAT) Coordinator

The Community Health Action Team (CHAT) Coordinator supports the CHATs, a partnership between Franklin County Public Health and local communities to address public health issues with community-based and evidence-informed strategies. Participates in the development and administration of planning focused on population-based concerns around health and wellness or disease prevention services designed to benefit local communities. Creates partnerships and communicates with community members to successfully implement and deliver evidence-informed programming.

Duties Include:

Develops, facilitates, and maintains community coalitions. Communicates and facilitates partnerships with agencies, schools, and/or organizations to identify community needs and promote active community health programs, education, and initiatives while integrating the social determinants of health and health equity.

Participates in planning and administering new program development (e.g., assesses community needs and proposes new programs, identifies possible barriers to programs, assists with budget development of programs, recommends policies and procedures, monitors implementation of projects, evaluates the effectiveness of projects, creates educational materials, trains staff and community)

Assesses the health needs of certain populations and communities, analyzes, and evaluates programs and policies, and uses the assessment information to develop plans to carry out the FCPH public health objectives. Guides community partners through health planning processes to improve local outcomes, develop recommendations for utilization of existing community resources and adoption of new action plans to improve health.

Coordinates grants and grant activities related to population health, increasing health education and promotion.

Communicates with staff, citizens, partners, and stakeholders to obtain or disseminate information related to community health planning and assessment. Facilitates ongoing communication and follow-up with community members.

Represents Franklin County Public Health at various public meetings, events, and with community groups. Provides information, literature, and technical assistance to groups concerning public health community projects, planning efforts, and/or other FCPH projects. Communicates with other agencies, schools and/or organizations to identify community needs and promote programs.

Provides guidance to organizational partners, public agencies, health care providers, institutes, and community residents on issues related to community health assessment and planning. Assures the alignment of community planning with FCPH’s health improvement and strategic plans. Provides data and status reports as needed.

Other related duties as assigned.

Requirements:

Bachelor's degree required.

2 years related experience and/or training.

Ohio Driver’s License.

Hiring Wage Range: $25.40/hour - $29.21/hour. This is a non-exempt position.
